Job Description

Position Title:   Case Manager  

Mission:   Committed to the elimination of racism and empowerment of women and children. 

Objective:    

This position plays a vital role in the success of the Housing Programs at the YWCA.  The empowerment of the women depends on the Case Manager’s ability to evaluate crisis situations, offer basic counseling, and make referrals based on each woman’s individualized needs and goals. The Case Manager will assist clients in recognizing both self-defeating and successful behavior patterns and connect them with the appropriate community resources during the intake process. The Case Manager must meet the following criteria:  

Responsibilities: 

  • Conduct intake assessments and develop Individualized Service Plan (ISP) for new clients. 

  • Engage regularly with clients, addressing needs and making community referrals. 

  • Attend staff meetings and participate in program discussions. 

  • Evaluate housing barriers for homeless individuals and families to determine needed services.  

  • Assist with housing procurement, financial planning and self-sufficiency strategies.  

  • Communication clearly with tenants, landlords, and agencies. 

  • Provide information and referral to social services and community programs. 

  • Help tenants create and manage personal budgets and establish payment plans, if needed. 

  • Develop individualized, strength-based, solution-focused goals for tenants to promote permanent housing and self-sufficiency. 

  • Conduct home visits to monito progress and offer ongoing support. 

  • Educate tenants on their rights and responsibilities under lease agreements. 

  • Maintain accurate tenant records, reports and documentation.  

  • Provide transportation services when necessary.  

  • Collect and report data, including HMIS reporting.  

  • Complete monthly and annual report forms. 

  • Stay organized with proper documentation and file management. 

  • Perform additional duties as assigned. 

Requirements: 

  • Bachelor's degree, with preference given to candidates who have experience serving the homeless population, individuals with mental health issues and/or those with substance use disorders.  

  • Reliable attendance, task completion, and performance. 

  • Strong organizational and communication skills. 

  • Proficiency with computers and Microsoft Office Software. 

  • Familiarity with tenant rights and strengths-based case management. 

  • Positive role model for clients in self-sufficiency and responsibility. 

  • Knowledge of issues such as racism, sexism, domestic violence, mental health, addiction, homelessness, and employment barriers.  

  • Professional conduct when representing the organization. 

  • Valid driver’s license.  

  • Stay informed on program funders. 

  • Attend all necessary meetings, including but not limited to Continuum of Care (COC), and Coordinated Entry System (CES). 

  • Join sub committees as requested by your supervisor. 

  • Make work-related purchases using the company credit card, as personal card use for work will not be reimbursed.  

Classification: Exempt, Salaried 

Hours: Monday through Friday. Full-time; minimum of 35 hours per week. 

Pay Range: $52,000-$56,000
