Job Summary
Make a meaningful impact in your community—join Acentra Health as a Clinical Evaluator (PASRR) in Port Angeles, Washington. In this hybrid role, you will conduct in-person assessments in nursing homes, hospitals, and community-based settings throughout Port Angeles and its surrounding areas.
As a Clinical Evaluator, you will conduct face-to-face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals with complex needs. Your primary focus will be completing clinical evaluations for individuals with Serious Mental Illness (SMI), Intellectual Disabilities (ID), or Related Conditions (RC) across nursing homes, hospitals, and community-based settings. . These evaluations are crucial in determining the next level of care, improving patient well-being, and supporting better health outcomes.
This full-time Clinical Evaluator position offers both flexibility and purpose—combining remote work from your home office with local travel. Acentra Health covers and reimburses all travel-related expenses, including mileage and parking. In addition, Acentra provides all necessary technology equipment for your home office, including a laptop for use in the field.
Job Responsibilities:
Required Qualifications, Knowledge, and Experience
Knowledge of public mental health systems and medical programs, including public sector services and supports, with a strong understanding of Level of Care guidelines and related standards of practice.
Expertise in assessing and analyzing psychological, emotional, behavioral, and physical functioning to inform clinical evaluations.
Experience formulating clinical diagnoses for complex psychological and behavioral health conditions.
Knowledge of Level of Care standards and best practices related to mental health, geriatrics, and long-term care needs.
Proficient in computer-based software, including Microsoft Office Suite (Word, Excel, Outlook, Teams).
Experience diagnosing psychiatric disorders using current DSM standards (state-specific) is a plus.

Compensation
The compensation for this role is $85,280 to $91,520 annual base salary
Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
