Job Summary:

At Call4Health, we believe that exceptional patient care starts with compassionate professionals like you. As a Remote Licensed Practical Nurse (LPN) , you’ll play a vital role in supporting patients and providers, helping to improve health outcomes, and easing the challenges of managing chronic conditions—all from the comfort of your home.

What You’ll Do:

• Respond to patient inquiries with empathy and accuracy through EHR and patient portals.
• Provide patient education on managing chronic conditions such as diabetes, hypertension, and COPD.
• Refill medications and coordinate follow-up care based on established protocols.
• Facilitate referrals and assist with prior authorization requests, ensuring seamless patient support.
• Monitor and interpret RPM results to deliver timely interventions and guidance.
• Support clinical trials by screening and educating patients.

What You’ll Need:

• A Licensed Practical Nurse Certificate and 2-4 years of clinical experience, ideally in primary care, OB/GYN, Orthopedics, Ambulatory Care, or Home Health.
• Proficiency with at least two EHR systems and an active, unencumbered eNLC or Enhanced Nurse Licensure Compact license.
• Strong teamwork, leadership, and organizational abilities, coupled with a compassionate approach to patient care.
