Job Description

Position Summary

The Residential Coordinator is responsible for case management and monitoring of residential settings, including the direct care providers of those services.  The Residential Coordinator ensures all care for individuals receiving services are provided as contracted and is an advocate for persons receiving services to meet individuals’ needs and desires while allowing them to gain further independence.  The Residential Coordinator is a representative of Our House Inc. and is expected to professionally represent the organization’s mission and purpose .  

Responsibilities

  • Assist with orienting host home providers on the culture and values of the organization as well as their responsibilities as independent contractors.
  • Assist with orientation and training of new host home providers and continuous training of current providers.
  • Provide support to contractors and individual specific training as needed.
  • Monitor services in residential settings to ensure individuals are receiving excellent care and treatment that meets their specific needs in a professional manner.
  • Act as advocate for individuals in services.
  • Perform regular monitoring of services with home visits and ensure both providers and own documentation is correct and performed to agency standard.
  • Maintain regular communication with host home providers, individuals receiving services, families, guardians, caseworkers, etc.
  • Provide feedback and evaluations to host home providers regarding their level of care, documentation, or other situations that may arise.
  • Create, train on, and assist in implementing ISSPs and BISSPs that focus on independence and expansion of life skills.
  • Oversee all documentation, including but not limited to: medication administration records (MARs), incident reports (IRs or GERs), daily log notes, ISSPs, BISSPs, comprehensive life reviews, health and safety assessments, etc.
  • Ensure individuals’ financial ledgers are utilized and documented correctly.
  • Establish and maintain positive relationships with outside agencies and professionals in the field.
  • Attend, contribute, and participate in meetings as necessary.
  • Assist in getting individuals to appointments and other events as necessary, to ensure continuity of care.
  • Respond to calls from individuals receiving services and host home providers regarding emergencies.
  • Be available and on call for persons receiving services and direct care staff, when assigned.
  • Perform general clerical duties to include but not limited to: photocopying, faxing, emails, filing, scanning, shredding, and answering phones.
  • Assist with and attend various agency events as needed.
  • Represent organization at agency fairs and other venues in a professional manner.
  • Assist in maintaining state audit compliance and license tracking.
  • Other duties as needed.

Requirements

  • Must take drug screen and pass.
  • Must be willing to take and pass background checks.
  • Must be in good physical, mental and emotional health.
  • Must be able to lift a minimum of 50lbs; if required.
  • Must be available after hours on call for person receiving services and/or provider issues.
  • Must have valid driver’s license a good driving record.
  • Must have a vehicle in good working order with insurance and registration.
  • Must have Medication Administration training or be willing to get it.
  • Must have a general knowledge and experience with Microsoft Office programs
  • Must have current CPR and first-aid certification.
  • Must be 21 years of age or older.

Qualifications

  • Experience working with or advocating for individuals with a variety of disabilities, including seizure disorders, individuals with behavioral needs, or non-verbal preferred.
  • Bachelor’s degree within the human service field desired; or equivalent experience.
  • Previous knowledge of Colorado I/DD rules and regulations and previous IRSS case management experience preferred.
  • Must be able to work independently and as a team.
  • Must be able to create and maintain a safe and effective environment, using best judgment.

  • Strong leadership skills and positive management of philosophies and practices.
  • Ability to identify and solve problems and communicate solutions through collaboration and application of strategic management.
  • Superior critical problem-solving skills and decision-making skills.

Salary $46,000
