What You'll Do:

Serve as a key clinical liaison to simplify and centralize urgent patient care needs.

Coordinate with referring providers to ensure patients receive the appropriate level of care.

Arrange transportation for patients, including emergency medical flights and ambulance services.

Collaborate with a multidisciplinary team to provide the best possible care for patients and their families.

What We're Looking For:

Education: Bachelor's Degree in Nursing (BSN) preferred.

Experience: At least one year of acute care medical/surgical nursing experience required. Experience in phone triage, discharge planning, or case management is a plus.

Licensing: Active RN license in the state where patients are receiving care.
